Time Off
Leave Time
Time off with pay must be scheduled in advance for vacation and holidays and can be used as needed for illness, illness in family, personal reasons, etc. It can be used as earned, after completion of the probationary period, or is valued at year-end for time off, accumulation of up to 520 hours, or cash-out as desired. A leave-time buy-back program is available for employees who wish to exchange available leave weeks for an increase in their base pay rate.

Part-Time Employees
Accruals on a per pay period basis are pro-rated based upon hours paid; accrual amounts increase based upon years of service in the same manner as for full-time employees above.
Leave of Absence
An excused absence from active duty lasting for a period in excess of 3 consecutive calendar days up to a maximum of 6 months in a 12 month period. Leave time may also be available on an intermittent basis for absence related to a "Serious Health Condition."
Planning for Your Future
Life Insurance
Hourly full-time and part-time employees scheduled to work at least 40 hours biweekly - Group Life and Accidental Death and Dismemberment Coverage each equal to one times annual base earnings.
Salaried employees - Group Life and Accidental Death and Dismemberment coverage each equal to two times annual base earnings.
Supplemental Life Insurance
Option to purchase Universal and/or Term Life Insurance for self and family members through payroll deduction.
Travel/Accident Insurance
Salaried Employees - One times annual salary while on hospital business.
Long Term Disability
Employees who fill a position authorized to work at least 20 hours per week (.5 FTE) and who have been employed for at least one month - 60% of salary after 3 months of disability up to a maximum of $9000 per month.
Short Term Disability
Option to purchase Short Term Disability Insurance through payroll deduction.
Pension Plan
The hospital provides a non-contributory Cash Balance Pension Plan for employee retirement planning. Participation requires age 21 and 1000 hours of service in Plan Year (July 1 - June 30). Hospital contributions equal 3.5% of base earnings. Employees have a 100% vested interest at 5 Years of Service in Plan.
Retirement Savings Plan (403B)
Tax sheltered savings are available for employee retirement planning. An investment by the employee, to a select group of funds managed by the Fidelity Investment Company, of up to 6% of pay, is matched by the hospital as follows:

Investments of 1% of salary are automatically begun for new employees unless the employee elects not to participate. Employees may invest up to the legal maximum. Free financial planning appointments with Serafini Financial Services, Inc. can be arranged through Human Resources.

Wages
Wage and Salary Program
Our salary schedules have been developed for each job classification and are regularly reviewed against rates being paid by other hospitals and industries in our area.
Performance Program
Employees are eligible for a performance wage review annually with wage increases based upon performance consistent with the wage and salary program.
Organizational Improvement Challenge
If the hospital achieves certain financial goals for the fiscal year as well as meets or exceeds performance measures for cost and patient satisfaction, employees may share in financial rewards as approved by the board of trustees annually.

On-call Pay
Hourly employees may receive $4.25 additional compensation for hours on-call. Actual hours worked are paid at one and one-half the base hourly rate. (Emergency call-in: minimum of 3 hours at time and one-half.)
Overtime Compensation
Hourly employees will receive payment at time and one-half for hours worked in excess of 40 per week. Salaried non-supervisory employees - payment at base rate for hours worked in excess of 40 per week may occur when employee is required to work two or more hours beyond the end of the scheduled shift or for an extra shift of two or more hours.
Premium Pay
Hourly employees may receive compensation at one and one-half times base rate for actual hours worked on a hospital recognized holiday.
Health and Wellness
Health Benefits
Option for employee and family members to participate in the hospital's group program with choice of two options: the Network Option (most services provided by network providers are paid at 100%) or the Basic Option (most services by any provider are paid at 80% after a $300 annual deductible).
Full-time employees - will receive a 90% allowance for their chosen option coverage. Full-time employees may opt not to participate in the plan and receive an additional cash payment of $29.24 biweekly.
Part-time employees - will receive a pro-rated allowance for their chosen option coverage; the pro-rated allowance will be between 50 and 90%, based upon the authorized biweekly hours of the employees position. On a semi-annual basis, the allowance will be recalculated based upon actual hours paid.
Pharmacy
A 10% Discount Program (for over-the-counter medical purchases) is available. A prescription drug benefit is also available as part of both the Network or Basic Option of the Health Plan - pharmaceuticals are provided with a $10/$15/$30 deductible payment.
Dental/Vision Insurance
Option for employee and family members to participate in the hospital's group program through payment of monthly premium.
Flexible Spending Accounts
Option for full-time employees, and part-time employees who fill a position authorized to work at least 40 hours biweekly, to deposit pre-tax monies into a health care or dependent child care account for tax-free reimbursement of eligible medical and/or dependent child care expenses. Reimbursement may be made through use of a Flex Convenience debit card.
Employee Health
Annual health assessment, physical, chest x-ray, immunizations and initial treatment for illnesses that develop after reporting on duty and not covered by insurance, are available without charge.
Employee Wellness Program
Participation in hospital sponsored wellness activities, including health screenings and smoking cessation programs.
Employee Assistance Program (EAP)
Confidential assistance for a wide range of personal problems, including marriage and family problems, alcoholism, substance abuse, financial and emotional problems of employees and immediate family members is available.
Other Benefits
Promotion & Transfer
Qualified employees are given an opportunity to express interest in position vacancies. Intra-divisional transfer/promotion is available after three months; intra-departmental lateral transfer is available immediately. Inter-divisional promotion or transfer is available after six months.
Educational Assistance Program
Tuition Reimbursement : All Full-time - Full reimbursement of tuition costs with maximum per semester cost determined annually. This benefit is available at completion of probationary period and completion of course with passing grade.
Part-time employees who fill a position authorized to work at least 40 hours biweekly or who have filled a regular position authorized at less than 40 hours biweekly but who, for the prior six months, have worked at least 40 hours biweekly - Pro-rated reimbursement of tuition costs with maximum per semester cost determined annually. (Pro-ration is based upon employee's worked percent of full-time.) This benefit can be applied for at any time but actual reimbursement is only available at completion of probationary period and completion of course with passing grade.
An academic assistance program is also available to all employees who are pursuing one of the specified degrees. The employee may apply for payment of tuition (up to a per year maximum) associated with their chosen program. It is expected that the employee will repay the borrowed amount through a service credit agreement with the hospital.
Child Care
The Learning Center has reduced rates for hospital employees who are scheduled to work a minimum of 32 hours biweekly (.4 FTE).
Parking
Free parking on designated lots. Reduced-fee parking at hospital parking deck available through payroll deduction for employees who work at the hospital location.
Service Awards
Choice of special award for each five years of continuous eligible service. Service credit equals one year for each Plan Year (July 1 through June 30th) that the employee works at least 1000 eligible hours or 1/2 year for each Plan Year that he/she is employed but works less than 1000 hours.
